TBSA is the share of a person’s total skin that a burn has affected, written as a percentage. A 20% TBSA burn means one-fifth of the skin is burned.

Doctors use a percentage rather than just naming the burned spots because location alone doesn’t tell the full story. A burn covering both legs is far more dangerous than a small burn on one hand. The percentage gives the whole care team one clear, shared number to work from.

TBSA is one of two measures that matter. The other is burn depth — how far down into the skin the burn reaches — which we explain below. Together, size and depth decide how severe the injury is. What Is the Rule of Nines?

The rule of nines splits the adult body into regions, each worth 9% or a multiple of 9. A clinician adds up the burned regions to get a quick TBSA estimate.

Here is how the adult body breaks down, per StatPearls’ clinical reference on the Rule of Nines:

  • Head and neck: 9%
  • Each arm: 9% (18% for both)
  • Front of the torso: 18%
  • Back of the torso: 18%
  • Each leg: 18% (36% for both)
  • Genital area: 1%

Those numbers add up to 100%. This method is fast and easy to remember, which is why crews use it in the field and in the ER.

For smaller or patchy burns, doctors turn to the palm method. The patient’s own palm plus fingers equals about 1% of their TBSA, per the Minnesota Department of Health. The team counts how many “palms” the burned area covers.

Importantly, the standard rule of nines does not apply to infants and small children — their heads count for more and their legs for less. That’s why doctors don’t use the plain adult version on a young child.

The Lund and Browder Chart: The Most Precise Method

The Lund and Browder chart is the most accurate way to measure a burn. It’s a detailed body diagram that adjusts the percentages by age and body proportion, so a child and an adult are scored on their own scales.

Burn centers rely on it, especially for children, because it is widely regarded as the reference standard for burn-size calculation. The rule of nines gives a quick estimate; Lund and Browder gives a reliable one.

That accuracy matters more than it may sound. A precise TBSA figure guides fluid treatment, surgery decisions, and long-term care planning. On a large burn, small errors mean too much or too little IV fluid — and both can harm healing.

Burn Depth Matters as Much as Size: “Thickness,” Translated

Percentage alone doesn’t tell the whole story. How deep a burn goes matters just as much as how much skin it covers. Doctors describe depth in three main levels — here’s what each clinical term actually means:

  • Superficial (first-degree): Affects only the top layer of skin. Think of a mild sunburn — red and painful, but it usually heals on its own. These are generally not counted in TBSA.
  • Partial-thickness (second-degree): Reaches into the deeper skin layers. Often blisters, very painful, and usually needs medical care. These count toward TBSA.
  • Full-thickness (third-degree): “Full-thickness” means the burn has destroyed every layer of skin. It may look white, leathery, or charred. Because the nerve endings are also destroyed, the burned area itself may not hurt — which can be frightening but is not a good sign. These are the most serious.

TBSA drives the most critical choices in the first hours after a burn. The biggest is fluid resuscitation.

Large burns cause the body to lose fluid rapidly through the damaged skin. Without replacement, blood pressure drops and organs can fail. Doctors use the TBSA percentage to calculate exactly how much IV fluid a patient needs. As a benchmark, burns greater than about 20–25% TBSA typically require IV fluid resuscitation, per U.S. HHS burn-triage guidance (CHEMM).

TBSA and depth also help decide whether a patient needs a specialized burn center. In plain terms, burn centers handle the most serious cases: large burns, deep burns, burns to the face, hands, feet, or genitals, and burns involving inhalation (breathing) injury. The bigger and deeper the burn, the longer and harder the recovery. A high TBSA often means multiple surgeries, weeks or months in the hospital, and a long rehabilitation road afterward.
