Anatomy: how the spine protects (and can endanger) the spinal cord

Normally, the bones shield the cord like armor. A violent fracture turns that shield into a threat. When a vertebral body shatters, sharp bone pieces can be pushed backward into the canal. There, they can bruise, press on, or tear the cord.

Doctors divide the spine into three regions:

  • The cervical spine is the neck.
  • The thoracic spine is the mid-back.
  • The lumbar spine is the lower back.

Where a fracture happens changes which parts of the body it affects.

What is a burst fracture?

A burst fracture is when the vertebral body is crushed from top and bottom. It then shatters outward in several directions. Doctors call the force axial loading. This is a heavy, straight-down squeeze that “bursts” the bone.

The danger is where the pieces go. In a burst fracture, bits of the vertebral body can push backward into the spinal canal, toward the cord. That’s what makes a burst fracture worse than a simple crack.

These fractures come from high-energy trauma. In the catastrophic-injury cases our attorneys handle, common causes include truck and car crashes, falls from a height, and other serious accidents. Burst fractures most often strike the thoracolumbar junction — around T11 to L1. This is where the stiff, rib-anchored mid-back meets the more mobile lower back, and forces build up.

Is a burst fracture a spinal cord injury?

Not always. Some burst fractures happen with no nerve damage (no neurological deficit). But because bone pieces can enter the canal, the risk to the cord is real.

Here’s the key point. The fracture is the broken bone. A spinal cord injury is damage to the cord itself. A burst fracture can happen with a healthy cord. Or it can crush the cord and cause partial or complete paralysis.

When bone pieces press on or tear the cord, the injury may be permanent. Doctors measure how complete a cord injury is. They use the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) and the ASIA Impairment Scale. This scale is published by the American Spinal Injury Association.

The scale runs from AIS A to AIS E. AIS A is complete — no movement or feeling below the injury. AIS E is normal. That single letter often shapes the whole talk about recovery and future care.

Warning signs of cord damage include numbness, weakness, and loss of bladder or bowel control. Any of these after an accident is a medical emergency.

Burst fracture vs. compression fracture vs. chance fracture

These three fractures differ in how the bone breaks. They also differ in how likely they are to harm the cord. Knowing the difference helps you understand a diagnosis.

Compression fracture

The front of the vertebral body collapses, like a wedge squished at the front. The back stays intact. Compression fractures are often more stable, and the cord is less often involved. Burst fracture

The whole vertebral body shatters, front and back, from a strong straight-down load. Pieces can spread into the spinal canal. This is the type most likely to threaten the cord.

Chance fracture

This is a horizontal break that pulls the vertebra apart, front to back. It’s often called a “seatbelt fracture.” It can happen in a crash when the body folds over a lap belt. Chance fractures are often linked to internal belly injuries. So trauma teams look closely for both.

So the difference between a burst fracture and a chance fracture is direction. A burst fracture crushes and explodes the bone outward. A chance fracture stretches and splits it sideways.

Stable vs. unstable fractures: why it matters

A fracture is “unstable” when the spine can no longer hold itself in safe alignment. That’s what makes cord damage more likely. Stability is one of the biggest reasons a broken back becomes life-altering.

Doctors often think of the spine in three columns: the front, middle, and back parts of each vertebra. This is called the Denis three-column model. When only one column breaks, the spine is usually still stable. When two or more columns fail, the spine can shift or collapse. That’s an unstable fracture.

Why does this matter to you? An unstable spine can move after the injury. That can cause nerve damage that gets worse hours or days later. In plain terms, the harm can grow with time. That’s why doctors treat unstable fractures urgently. They act fast to hold the spine in place and protect the cord.

Symptoms and warning signs of spinal cord involvement

Severe back pain after a fall or crash is a red flag. But the nerve signs matter even more. These suggest the cord — not just the bone — is involved.

The Mayo Clinic says a spinal cord injury can cause loss of movement and feeling below the level of injury. Warning signs include:

  • Numbness or tingling in the arms, legs, or torso
  • Weakness, or not being able to move a limb
  • Paralysis below the level of the injury
  • Loss of bladder or bowel control
  • Trouble breathing, with higher (neck) injuries

Do not “wait and see.” Any of these after an accident calls for emergency care. Moving someone with a suspected spinal fracture the wrong way can make the injury worse. That’s why paramedics stabilize the neck and back before moving a patient.

What is a T12 fracture, and why location matters

A T12 fracture is a break in the twelfth thoracic vertebra. It sits at the fragile thoracolumbar junction, where burst fractures are common. Because of this spot, a T12 injury can affect the legs, lower body, and bladder and bowel function.

The level of the fracture drives which functions are at risk. The Mayo Clinic explains that the higher up the injury sits, the more of the body it can affect. A neck (cervical) injury can affect the arms, legs, and even breathing. A lower injury, like T12, usually affects the legs and lower body.

Diagnosis and treatment overview

Doctors diagnose these fractures with imaging. They treat them based on stability and nerve involvement. The goal is always to protect the spinal cord.

Imaging usually starts with an X-ray. A CT scan shows the bone detail and where pieces have gone. An MRI shows the cord and soft tissue. This lets doctors see if the cord is being pressed on.

Treatment falls into two paths:

  • Stable fractures with no nerve damage may be managed with a brace, rest, and close monitoring. The brace is often a TLSO (thoracolumbosacral orthosis).
  • Unstable fractures, or when the cord is at risk, often need surgery. Surgeons may do decompression (taking pressure off the cord). They may also do stabilization or fusion (locking the bones together with rods and screws to hold the spine steady).

How long does a spinal fracture take to heal?

Bone healing from a spinal fracture usually takes several weeks to a few months. It varies from person to person. But here’s the part that matters most: the bone can heal while spinal cord damage stays permanent.

That difference is easy to miss. A vertebra may knit back together in three months. But if the cord was crushed, that nerve damage may never reverse. Recovery from a spinal cord injury is a longer, far less predictable process.
