But inside, you’re wondering: are those two problems, or one? Which one heals? Which one is why he can’t move his legs?

You are not confused because you missed something. In the first days, hospital teams use “fracture” and “cord injury” almost like they mean the same thing. That’s because in a serious crash, both often happen at once. They form a column. The spinal cord runs down the middle of that column. It is a bundle of nerve tissue about as thick as your thumb. It carries signals between your brain and the rest of your body, according to the Mayo Clinic.

Think of it as a cable inside a run of protective pipe. A fracture damages the pipe. A cord injury damages the cable.

This is why three very different situations exist:

A fracture with no cord damage — the pipe cracks, but the cable is untouched.

A cord injury with no fracture — the cable is hurt by stretching, swelling, or pressure, even though the pipe held. This is more common in older adults and in violent trauma than most families expect.

Both together — the most common pattern in truck crashes and high-force falls.

Where the cord is hurt matters as much as whether it’s damaged. The cervical spine is your neck. The thoracic is your mid-back. The lumbar is your lower back. The higher up the cord an injury sits, the more of the body it affects, the Mayo Clinic notes. Our guide on spine injury levels shows how “C5” or “T10” translates into function.

What a spinal fracture actually is

A spinal fracture is a break or crack in one or more vertebrae. A healthy spine takes tremendous force to break. That’s why these show up in the catastrophic cases our attorneys handle — commercial truck crashes, falls from height, and violent assaults.

Doctors classify the serious ones by how the bone failed:

Compression fracture — a vertebra collapses front-to-back under load.

Burst fracture — a vertebra shatters and can drive bone fragments toward the cord.

Flexion-distraction (Chance) fracture — the spine is pulled apart, often from a lap belt in a high-speed crash.

Fracture-dislocation — a vertebra breaks and shifts out of place. This is the most unstable pattern.

To see all this, the care team uses several scans. An X-ray shows bone alignment. A CT scan shows the break in detail. An MRI shows soft tissue — including whether the cord itself is bruised, squeezed, or torn. The MRI is usually the scan that answers the question you really care about: is the cable hurt?

What a spinal cord injury actually is

A spinal cord injury is damage to the cord itself. It disrupts movement and feeling below the level of injury, the Mayo Clinic explains.

Doctors describe how bad it is with two words you’ll hear a lot:

Complete — no movement or feeling below the injury.

Incomplete — some function or feeling remains.

The grading system behind those words is the ASIA Impairment Scale. It’s published by the American Spinal Injury Association. Its grades run from A (complete) to E (normal). So when a doctor says “he’s an ASIA B,” that’s what they mean — a precise, standard way to describe how much of the cable still works.

Here is the hard truth that shapes everything else. A broken bone can heal. A damaged spinal cord usually cannot. That is why cord injuries cause permanent paralysis — paraplegia (loss of function in the lower body) or tetraplegia/quadriplegia (loss of function in all four limbs).

The National Spinal Cord Injury Statistical Center (NSCISC) keeps the country’s largest SCI database. It estimates that roughly 18,000 new spinal cord injuries occur in the United States each year. Vehicle crashes are the leading cause. The Christopher & Dana Reeve Foundation tracks how many Americans live with paralysis of some form.

How a fracture and a cord injury connect

A spinal fracture can injure the cord — or leave it completely alone.

When it does cause harm, it’s usually mechanical. Bone fragments from a burst fracture push inward. A dislocation shifts bone against nerve tissue. Or swelling in the hours after the crash presses on the cord. That last point is why this matters so urgently.

An unstable fracture is an emergency, even if no paralysis has appeared yet. An unstable break can still shift and injure a cord that’s currently fine. The care team may brace, immobilize, or surgically stabilize the spine. They do this to protect the cable before it’s damaged.

Here is the sentence families most need to hold onto. The lasting damage almost always comes from the cord injury, not the broken bone. A vertebra that heals cleanly may leave few long-term effects. Cord damage is what drives lifelong impairment — and lifelong cost.

Does a fractured spine mean paralysis?

No. Many spinal fractures heal without permanent paralysis. Whether paralysis happens depends on one question. Was the cord or nerve roots damaged, and how badly?

The Mayo Clinic lists warning signs that the cord may be involved: loss of movement, numbness or changed feeling, and loss of bladder or bowel control. These are emergencies. Fast treatment can matter for the outcome.

Have questions about what happened?

Ask our team directly. If the fracture is stable and the cord is spared, many people walk again after treatment or bracing. Walking is at risk when the cord or nerves are damaged, or when the fracture is unstable.

Please don’t try to judge this yourself. Two people can have the exact same broken vertebra and completely different futures. The difference is in the cord, not the bone. Only your treating team can tell you what your injury means. They use imaging and neurological exams to figure it out.

Does a spinal fracture ever heal?

Yes. Many vertebral fractures heal over weeks to months with bracing, rest, or surgery. Healing depends on the fracture type, whether the break is stable, and the patient’s age and health.

But healing bone is not healing nerves. A vertebra can mend perfectly while the cord damage stays permanent. That gap — between a “healed” spine on an X-ray and a life permanently changed — is exactly what confuses families. It’s also exactly why a fracture and a cord injury must be understood separately.

What is the worst kind of spinal injury?

Generally, the most severe are high cervical, complete cord injuries. The reason is location. The higher on the cord, the more of the body it controls. An injury at the top of the neck (around C1–C4) can affect the muscles that let a person breathe, the Mayo Clinic notes. Sometimes this requires a ventilator.

Some catastrophic spinal injuries are fatal. We say that plainly and with care, because families deserve honesty. When a loved one dies from an injury caused by someone else’s wrongdoing, the family may be able to bring a wrongful-death claim. Nothing undoes the loss. But a claim can help a family cover costs and hold a wrongdoer accountable.

The lifetime cost of a catastrophic cord injury

Cord injuries reshape daily life permanently. They can cause paralysis, loss of feeling, chronic pain, and problems with the bladder, bowel, breathing, and other systems.

The financial weight is enormous and lifelong. Per the NSCISC, the first-year and ongoing costs of severe SCI can reach into the millions of dollars over a lifetime. These costs are driven by:

Ongoing medical care and rehabilitation

Wheelchairs and special equipment

Home and vehicle changes

Lost income and lost earning power

Long-term personal care and attendant support

This is why documenting the full future impact is central to a serious claim. In the cases our attorneys handle, the true cost is measured across decades, not weeks. A claim that ignores future needs can leave a family short of what care actually requires.

Are you comparing a cord injury to a related but different problem? See our pages on herniated discs and nerve damage and cauda equina syndrome, a nerve emergency that can look like a cord injury.

When someone else’s negligence caused the injury

If a careless or wrongful act caused a catastrophic spinal injury, the injured person or family may have a legal claim. Two situations come up again and again:

A truck crash. A loaded commercial truck can carry 20 to 30 times the mass of a passenger car. That force can shatter vertebrae and damage the cord in a single impact.

An attack on someone else’s property. Suppose you were assaulted, shot, robbed, or otherwise hurt in an apartment complex, a parking garage, a hotel, a gas station, or a store. And suppose it happened because the owner failed to provide reasonable security — like working locks and gates, lighting, cameras, or guards. You may have a claim against that property owner. The legal name for this is negligent security.

Two Florida rules are worth knowing:

Deadlines apply. For most negligence claims that start on or after March 24, 2023, Florida generally allows two years to file suit under Fla.
