One is scared and in pain, searching for answers late at night. The other is a spouse, parent, or adult child who is watching a loved one. That person may be asking, Is this an emergency, or am I overreacting?

Trust that worry. Maybe the person you care about cannot feel their bladder filling. Maybe they cannot control their bowels. Maybe they have gone numb between the legs. If so, you are not overreacting. These are not signs of a “bad back.”

At triage, say the words plainly. For example: “I’m worried about cauda equina. There’s new bladder trouble and saddle numbness.” Naming your concern can help staff understand how urgent it is.

Quick answer: The warning signs of cauda equina

Cauda equina syndrome happens when a bundle of nerve roots at the base of the spine is squeezed, or compressed. These nerves control the legs, bladder, bowel, and sexual function. When they are compressed, those functions begin to fail.

Signs that cauda equina may be the cause:

You suddenly cannot start urinating, or you cannot feel your bladder filling.

You lose control of your bladder or bowel, or you leak without knowing it.

The area you would sit on in a saddle feels numb or tingly.

One or both legs feel weak, heavy, or numb.

You have severe or worsening low back pain. Sometimes the pain shoots down both legs (sciatica).

If several of these appear together, call 911 or go to the nearest emergency room.

What is cauda equina syndrome?

Cauda equina syndrome is compression of the nerve roots at the bottom of the spinal cord. “Cauda equina” is Latin for “horse’s tail.” The roots fan out below the cord and look like one.

These nerves carry signals to and from the legs, bladder, bowel, and pelvic area. Anything that presses on them can block those signals. The most common cause is a large herniated disc at L4–L5 or L5–S1. Other causes include a tumor, an epidural abscess or other infection, spinal bleeding, or trauma.

As the Mayo Clinic explains, spinal nerve damage can cause loss of movement, changed sensation, and loss of bowel or bladder control.

CES is uncommon. The most cited estimate is about 1 in 33,000 to 1 in 100,000 people per year (StatPearls, National Library of Medicine, NCBI Bookshelf). When it is missed or treated too late, it can cause permanent paralysis. It can also cause lasting loss of bladder, bowel, and sexual function.

Doctors treat CES as a surgical emergency, not as ordinary back pain. Compressed nerves can handle pressure for only so long. After that, permanent damage can set in.

The red-flag signs, one by one

The red flags involve the bladder, bowel, saddle area, and legs. They are not only back problems. Ordinary back pain does not cause them.

Bladder trouble. You cannot start urinating, you do not feel your bladder fill, or you leak. Doctors call this urinary retention or incontinence. Retention is often the earliest true CES sign.

Bowel changes. You lose bowel control. Or you lose the feeling that tells you when you need to go. Doctors call this fecal incontinence.

Saddle numbness. The groin, buttocks, and inner thighs feel numb or tingly. Doctors call this “saddle anesthesia.”

Leg weakness or numbness. One or both legs feel weak, heavy, or numb. You may trip or struggle to stand.

Bilateral sciatica. Pain shoots down both legs, not just one. This is more concerning than pain on one side.

Loss of sexual sensation. You have new numbness or loss of function in the genital area.

Symptoms can appear over hours. They can also build over days. In either case, the bladder, bowel, and saddle signs are the ones that turn back pain into an emergency.

What does cauda equina feel like?

Many people describe the lower body going numb or “switching off.” Back pain is worse than usual. The legs feel heavy or unresponsive. Some people cannot tell when their bladder is full.

The saddle area may feel as if it has been numbed at the dentist. Wiping after using the bathroom may feel strange, or you may feel nothing at all. Sitting on a hard chair may feel oddly muffled.

Early on, CES can feel like a rough day with a bad back. That is why people, and sometimes providers, explain it away. A bad back does not stop you from feeling your bladder or controlling your bowel. When those functions change, the change is your body’s alarm.

Have questions about what happened?

Ask our team directly. The MRI is the definitive test. The goal is fast confirmation, so surgery can relieve the pressure quickly.

A typical workup includes:

A neurological exam. The doctor checks reflexes, strength, and sensation in the legs and saddle area. They may check anal tone and sensation around the groin. These areas map directly to the affected nerve roots.

The straight leg raise test. You lie flat while the doctor lifts your straightened leg. This checks for nerve-root irritation.

A bladder scan (post-void residual). A quick ultrasound measures how much urine remains after you urinate. A large amount left over is a key CES clue.

An urgent MRI. Imaging shows exactly where and how badly the nerve roots are compressed. If MRI is not available, a CT myelogram is the backup.

When imaging confirms compression, surgeons often perform lumbar decompression (a discectomy or laminectomy). This takes pressure off the nerves. Treatment guidelines stress speed. The longer the nerves stay compressed, the higher the risk of permanent damage.

What could be mistaken for cauda equina?

Cauda equina is most often confused with ordinary sciatica, a herniated disc without nerve-root compression, or lumbar spinal stenosis. All of these cause back pain and leg symptoms, so they overlap. It can also be confused with conus medullaris syndrome, which involves compression slightly higher on the spinal cord. Less often, it resembles a multiple sclerosis flare or peripheral neuropathy.

The red flags help tell these apart. A herniated disc can hurt badly and shoot pain down one leg. But it usually does not cause bladder retention, bowel changes, or saddle numbness. Spinal stenosis can make walking hard. It rarely causes sudden loss of bladder control.

Look for back or leg pain that appears with bladder, bowel, or saddle changes. That combination calls for urgent imaging. Missing it is how a treatable condition becomes a catastrophic one.

Why timing matters

Compressed nerves can be permanently damaged if pressure is not relieved quickly. Much of the surgical research stresses decompression within about 24 to 48 hours after red-flag symptoms begin. Many surgeons aim to operate as soon as possible once CES is confirmed.

Delays like these can happen:

A patient is sent home from the emergency room with pain medication.

An MRI is never ordered, even though saddle numbness was documented.

Surgery is postponed overnight.

When the window is missed, the outcome can include permanent paralysis, lifelong incontinence, and loss of independence.

The National Spinal Cord Injury Statistical Center tracks U.S. spinal cord injury data. The American Spinal Injury Association publishes the ISNCSCI scale. Doctors use this scale to grade how severe and complete a spinal injury is. When CES leads to lasting paralysis, the Christopher & Dana Reeve Foundation describes the daily reality families then face.

When a delayed or missed diagnosis may have caused lasting harm

Sometimes a provider misses documented red flags. The provider may fail to order an urgent MRI or delay surgery. If permanent harm follows, the family may have legal options. This type of claim is a form of medical negligence.

Not every bad outcome is negligence. Some cases move too fast for anyone to stop the damage. The question is different when red-flag signs were recorded and no one acted on them. These claims concern permanent, life-altering outcomes, such as paralysis, bladder and bowel loss, and lasting nerve damage. They do not concern ordinary back pain.

Florida sets deadlines for filing injury claims. Medical-malpractice claims follow their own rules. These deadlines can run quickly. It helps to speak with a lawyer early so you understand where you stand.

Talk with CHG Personal Injury Lawyers

CHG Personal Injury Lawyers is a Florida-based personal injury firm.
