It links your brain to nerves all over your body. Think of it as the body’s main cable.

A serious accident can bruise, squeeze, or tear that cable. When the nerve fibers are damaged, signals that used to flow freely get blocked or scrambled.

This is why one injury can cause two very different problems at once. Some signals never arrive. So you lose movement and feeling. Other signals fire wrongly. So you feel strange or severe pain.

In the catastrophic-injury cases our attorneys handle, families are often shocked. A single moment can change the body this completely. Understanding the “why” behind these changes can bring a little clarity during a frightening time.

Complete vs. incomplete spinal cord injuries

The key difference is whether any signals still pass below the injury.

In a complete injury, no signal gets through. You lose all movement and feeling below that point. In an incomplete injury, some signals still pass. So some movement or feeling may remain.

Doctors don’t guess at this. They use a standard exam called the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI). It was published by the American Spinal Injury Association.

The exam gives an ASIA Impairment Scale grade. It runs from A (complete — no movement or feeling below the injury) to E (normal). If you see letters like “AIS C” in medical records, that is this scale. It tells the care team how complete the injury is. It also shows the lowest spinal level that still works normally.

That level matters. A cervical (neck) injury can affect the arms, hands, trunk, and legs. A thoracic (mid-back) or lumbar (lower-back) injury usually affects the legs and lower body. According to the National Spinal Cord Injury Statistical Center (NSCISC), incomplete tetraplegia is the most common type of new injury.

You may also hear two words that describe the result:

Paraplegia — paralysis affecting the legs and lower body.

Quadriplegia (tetraplegia) — paralysis affecting the arms and legs.

We explain these in more depth on our paralysis, paraplegia, and quadriplegia page.

Why a spinal injury causes paralysis

A spinal injury causes paralysis because the brain’s commands can no longer reach the muscles below the damaged area. Your brain still sends the signal to move. But it hits a broken section of the cable and stops.

The result is not only lost movement. Depending on the level, a person may also lose feeling. They may lose control over the bladder, bowel, breathing, and other automatic functions, per the Mayo Clinic. Higher injuries usually affect more of the body.

People often search for what a specific level means. A T6 injury sits in the mid-back. It usually affects the legs and part of the trunk. But it often spares the arms and hands. Even so, two people injured at the same level can have very different abilities. Only your medical team can review your case and explain what your injury means for you.

Why chronic pain happens: neuropathic vs. musculoskeletal pain

Chronic pain after a spinal injury usually comes from two different sources. Telling them apart matters, because they are treated differently. Doctors call them neuropathic pain and musculoskeletal pain.

Neuropathic (nerve) pain

Neuropathic pain comes from the damaged nerves themselves misfiring. They send pain signals even when nothing is hurting at that spot. People describe it as burning, stabbing, electric, pins-and-needles, or a deep squeezing. Here is its most confusing feature: it can appear in areas that feel numb or paralyzed.

If your skin feels nothing to the touch yet still burns, you are not imagining it, and it is not “in your head.” It is a known medical result of nerve damage. The Model Systems Knowledge Translation Center is a federally funded resource for spinal cord injury patients. It calls neuropathic pain one of the most common and hardest-to-treat problems after this kind of injury. This is why ordinary painkillers often don’t touch it. Instead, doctors reach for medicines that aim to calm nerve signaling.

Musculoskeletal pain

Musculoskeletal pain is different. It comes from real strain on muscles, joints, and bones.

After a spinal injury, your posture, movement, and daily tasks change. Pushing a wheelchair, moving in and out of it, and shifting weight put new, heavy stress on the shoulders, arms, wrists, and back. Over months and years, that stress causes aching, worn joints, and sore, overworked muscles.

Unlike nerve pain, it usually hurts where the strain is. It also eases with rest, position changes, or physical therapy.

Both kinds of pain can be constant and life-altering. They can last long after the original injury has “healed.” Chronic pain is one reason a spinal cord injury shapes daily life for decades, not months. It is also one reason the long-term costs are so high.

Have questions about what happened?

Ask our team directly. What the science says

The honest answer is that nerve fibers inside the spinal cord have very little ability to regrow. So full recovery from a serious injury is often not possible. Unlike some nerves in the arms and legs, spinal cord fibers do not heal well once torn, according to the Cleveland Clinic.

Still, “no recovery” is not the whole story. With incomplete injuries, some function can return. This is most likely in the first weeks and months. Rehab can help people regain strength and independence through the nerve pathways that survived.

So is a spinal cord injury reversible? For a complete injury, current medicine cannot reverse it. Can you walk again after a complete injury? Usually not. But outcomes with incomplete injuries vary widely from person to person.

We share this plainly because false hope helps no one. Researchers worldwide keep studying nerve repair, and progress is real. But no one can promise a specific recovery. Medical questions belong with your treating doctors.

How chronic pain and paralysis are managed

Chronic pain and paralysis are managed with a team approach. It combines medication, therapy, equipment, and emotional support. There is no single fix. Care focuses on comfort, function, and quality of life over the long term. A typical plan includes:

Medications to calm nerve pain, muscle spasms, and other symptoms.

Physical and occupational therapy to build strength and relearn daily tasks.

Assistive devices — wheelchairs, braces, and home equipment.

Mental-health support for the very real emotional toll.

Most people work with a team of specialists who plan care for years. The team also watches for serious complications. These include pressure injuries, muscle stiffness (spasticity), and a dangerous blood-pressure spike (autonomic dysreflexia). The Christopher & Dana Reeve Foundation describes these as lifelong management issues. Preventing them is an ongoing job.

This kind of care is expensive and central to living well. That leads to a question many families face: how will we pay for all of it?

The lifelong cost of a spinal cord injury — and who may be responsible

A serious spinal cord injury often demands decades of medical care, equipment, home changes, and lost income. The Christopher & Dana Reeve Foundation documents how paralysis affects millions of Americans and their families. When someone else’s carelessness caused the injury, a legal claim can help pay for that lifetime of care.

Truck crashes. A fully loaded commercial truck can weigh many times more than a passenger car. A single crash can cause catastrophic spinal injuries. Our truck accident page explains how these cases work.

Being hurt during a violent attack on someone else’s property. Maybe you were assaulted, shot, robbed, or attacked. It may have happened in an apartment complex, a parking garage, a hotel, a gas station, or a store. Maybe the property owner failed to provide reasonable security — working locks and gates, lighting, cameras, or guards. If so, you may have a claim against that owner. Lawyers call this negligent security. Learn more on our negligent security page.

Florida law shapes these claims in important ways. A negligence-based injury case generally must be filed within two years for claims arising on or after March 24, 2023, under Fla.
